LIST OF IMPORTANT DOCUMENTS AND TIPS
Important information that every family should know
- Names, phone numbers, and birthdates of all family members
- Phone numbers for your children's school, doctor, and workplace (if applicable)
- Create a list of allergies, medical conditions, and medications for everyone in your household (if applicable)
- Phone number for the Consulate of your country of origin
- Phone number for an Immigration Attorney
Important documents that should be kept in a safe place
- Birth and marriage certificates (if applicable)
- Passport from your country (for each person)
- Vaccination card (for children)
- Social security card (if applicable)
- Identification card
CAREGIVER AUTHORIZATION AFFIDAVIT
This form can be signed and kept with other important documents and is written to take effect in the event of a parent's detention. It allows a parent to designate an adult over the age of 18 as a "person in parental relation" and authorizes that person to make health and educational decisions for the child for periods of up to twelve months. It does not require going to court to complete this. It must be signed by the parents and the designated caregiver.
Powers Granted by the Caregiver Authorization Affidavit
- Review school, medical, and other records;
- Enroll the child in school;
- Excuse school absences;
- Consent to participate in school programs and/or school-sponsored activities;
- Consent to medical care related to school;
- Enroll the child in health plans;
- Consent to vaccinations;
- Consent to general medical care;
- Consent to medical procedures;
- Consent to developmental assessments; and/or
- Consent to mental health examination and/or treatment.

- Allows ICE to enter the home
- Signed by a judge
- It will have the name of a state or federal court at the top, such as "Supreme Court of the State of California."
→Administrative Order
- Does Not Allow ICE Entry
- Signed by an ICE supervisor, not a judge
- May have "U.S. Department of Homeland Security" at the top
